Online Game and Discipline Report below is strictly for LISA youth Matches. U11-18 Division 1 to Division 4

Game reports are due within 48 hours if there was no misconduct or discipline issues in the game. If there is misconduct to report (yellow and red cards) then the game report is to be submitted within 24 hours.
If the game was normal and had no misconduct to report, the body of the game report can be as simple as “no issues or concerns to report.” There is no need to report goal times or who the scorer was or even a description as to how each goal was scored.

Best Practice for showing a yellow or red card:

  • Identify the player
  • Isolate the player
  • Show the yellow or red card
  • Record the information in your notebook
  • Use the “stop sign” approach, if required, to give a clear signal to players to stop coming towards you


What should the referee Include in a Yellow Card (Caution) game report:

For yellow cards, include the reason and a brief description of the offense. Reporting cautions also requires the following information:

  • Team
  • Who committed the offense (name and #)
  • Reason for the caution from the drop down box
  • Factual description of the offense
    • Example: In the 65th minute, #7 [insert individual’s name] on [insert team name] was cautioned for committing a reckless tackle.

Red Cards:
Sending off offences require considerably more detail. This more detailed level of information is required in order to allow the Discipline Committee to determine the severity of the sanction and support the length of the suspension.

What should the referee include in a Red Card (Send Off) game report?

Red card game reports should be clear and concise; complete with all of the required information; and factual,
meaning avoiding opinions. Paint a detailed picture of what happened. Following the “Who, What, Where, and When” method will help ensure all of the required information is included in the game report.

WHO? WHAT? WHERE? WHEN?

  • Team
  • Player / Technical Staff Name (Coach; Manager; etc)
  • Player Number
  • The minute of play or if the offence occurred pre or post game
  • Point of Contact (what was the point of contact the player used to make contact with their opponent and where on the opponent did they make contact?)
  • Speed / Intensity (Low-Medium-High)
  • Force (Low-Medium-High)
  • If the send-off was for offensive, insulting, abusive or language or racial remarks; and/or action(s) use direct,
    uncensored quotes as it applies to players; coaches; supporters.
  • Remember to use language from the Laws of the Game
    (Law 12=Fouls and Misconduct) !
